RESPONSIBILITIESConductan initial in-home assessment. If there are no safety or wellbeingconcerns, case managers may conduct subsequent monthly engagementseither in-person or virtually but must complete an in-person homevisit at least every 90 days.Conduct ongoingassessments, make safety plans, create strengths-based plans withchildren and their sponsors, and refer and connect children tocommunity-based services.Carry a minimumcaseload of 15, if regionally based, or 8, if cases require traveloutside the area.Provide and foster acoordinated case management approach that adopts a collaborativeprocess of assessment, planning, facilitation, care coordination,evaluation, and advocacy for options and services to meet theunaccompanied child and the sponsor's presentingneeds.
