Job details
Care Coordinators will:
- Review patient records to ensure that information is accurate
- Ensure that patient records/documents accurately reflect the current conditions/treatment requirements of each patient.
- Provide preventative care by identifying care gaps
- Identifying Chronic Conditions Requirements
- CMA/LPN certificate/degree
- Ability to work full-time (in office)
- Empathetic and active listener
- Detail oriented
- Effective time management skills
- Strong communication and interpersonal skills (written and verbal)
- Two (2) weeks paid time off (PTO)
- Health Insurance
- Paid Holidays for the six (6) primary legal Holidays
- Professionalism and positive attitude
- Strong Work Ethic
- Must have 1 years or more supervisory experience
- Excellent communications skills both written and verbal
- Ability to prioritize and multitask quickly while maintaining attention to detail
