LPNSchool NursingServicesPre-SchoolStudent - with seizure concerns and requires aninhalerLPNwill ride to and from school with the student from home at thefollowing times: 7:35 AM Pick UpTime 3:50 PM Drop OffTime*PediatricExperienceRequiredServiceswill be rendered to student receiving instruction in the SchoolDistricts schools; private/parochial schools; or home setting.Services may be required to be provided on either an individual orgroup basis and in accordance with each students individualizededucation program ("IEP"). Proposers are requiredto be licensed and qualified to performthe services. Alltherapists performing services for the School District shall belicensed under the laws of the State of NewYork, inclusive of the State EducationDepartment Licensing requirements.Servicesshall consist of, but not limited to thefollowing:a.Service based on individual studentIEPs;b. The implementation ofgoals and objectives for the educational services provided as pereach individual students IEP;c.The completion of progress reports regarding student achievement ofobjectives as per report cardschedules;d. Provide theCommittee on Special Education (CSE) with annual progress reportsfor each individual student receiving service to be reviewed ateach students CSE meeting;e.Complete evaluations as per the request of the Director of SpecialEducation on an as neededbasis;f. Consult with educationaland other related services staff to maximize intervention andensure carryover of treatmentgoals;g. Upon referral, thetherapist shall administer individual evaluations to determine theindividual students need for therapy as a relatedservice;h. In connection withservices rendered to Medicaid-eligible students, provide attendancerecords, notes andassistance to the School District for Medicaidbilling;i. Attend CSEmeetingsRequirementsAlltherapists performing services for the School District shall belicensed under the laws of the State of NewYork, inclusive of the State EducationDepartment Licensingrequirements.*PediatricExperience RequiredAll therapists performing services for the School District shall belicensed under the laws of the State of New York, inclusive of theState Education Department Licensing requirements. *PediatricExperience Required INT
