Rotating Equipment Engineer

Care Coordinators will:
Review patient records to ensure that information is accurate
Ensure that patient records/documents accurately reflect the current conditions/treatment requirements of each patient.
Provide preventative care by identifying care gaps
Identifying Chronic Conditions Requirements
Requirements:
CMA/LPN certificate/degree
Ability to work full-time (in office)
Empathetic and active listener
Detail oriented
Effective time management skills
Strong communication and interpersonal skills (written and verbal)
Evening Hours: (5:30 pm- 1:00am) or (6pm-1am) - In Office ONLY
