You'll be a natural relationship builder, honest, helpful, and dependable, making you a great asset to our team.The Ideal Candidate: The ideal candidate will have at least 1 year of experience managing client enquiries, consultations, onboarding, and a client case load for Home Care Package, NDIS, and private pay clients. The Care Coordinator will have experience managing the complex scheduling requirements for community care workers.Purpose of Position: This role is customer-focused and involves consulting with families, preparing in-home care assessments, and drafting care plans to best meet client requirements. This position is responsible for recruiting, training, and managing the performance of our carers and nurses.Duties include:Assisting with handling a high volume of incoming telephone and internet enquiries.Scheduling services for clients by rostering care workers based on their qualifications and availability, creating master rosters, and managing daily changes.Meeting with clients and assessing their needs, handling information requests, feedback, general enquiries, and complaints.Preparing in-home care assessments and drafting care plans to best meet client requirements.Recruiting, training, and managing the performance of the care team.Maintaining client and care worker compliance and documentation according to government standards.Ensuring all documentation is accurately completed in Right at Home databases and software programs in accordance with organisational policies and procedures.Providing "on-call" services on rotation after hours, on weekends, and public holidays.Rewards Program: Exclusive to Right at Home staff through Rewards Gateway, including discounts on a variety of services (e.g., shopping and brand names).Working Hours: This position is Monday to Friday, predominantly 8:30 am - 4:30 pm (can be flexible).Note: No Working From Home.Job Type: Permanent Part-time, 37.5 hrs/week.Salary: $80,000 - $85,000 per year.QualificationsCurrent registration with AHPRA (RN/EN) OR Certificate III/IV in Individual Support (Ageing, Disability) or equivalent.Current Australian (Satisfactory) National Police check, Working with Children Check.Current First Aid & CPR certificate.Driver's license and your own reliable registered vehicle.Excellent verbal and written communication skills.Professional Indemnity Insurance (RN/EN).A smart mobile phone (iPhone / Android).Preferred SkillsAt least 1 year experience as a scheduler, case manager, care coordinator, hospital discharge planner, or NDIS support coordinator.Solid knowledge in the Disability and/or Aged Care sector(s).Enrolled Nurse or Registered Nurse will be an advantage.The ability to operate efficiently and deliver excellent service under pressure.Time management, organisational skills & ability to manage competing priorities.Relationship development skills to support diverse clients to achieve their goals and objectives.Intermediate to Advanced Microsoft Office experience (Excel, Word, Outlook, and CMS).
