Job Title: Field-Based Care CoordinatorLocation: Delhi, BangaloreJob Description:We are seeking a dedicated and compassionate Field-Based Care Coordinator to join our team. As a Care Coordinator, you will play a crucial role in facilitating the seamless execution of our program, ensuring effective communication between healthcare professionals (HCPs), patients, and our organization. This position requires excellent organizational skills, attention to detail, and a commitment to delivering high-quality patient care.Key Responsibilities:* Collaborate with medical representatives to onboard doctors as part of the program.* Facilitate briefing and introduction sessions for healthcare professionals (HCPs) according to standard scripts provided by the client.* Ensure successful patient onboarding onto the program.* Adhere to monthly clinic coverage calendars provided by the client, ensuring alignment and coordination.* Collect HCP onboarding details and consent, and execute HCP engagement plans effectively.* Enroll and onboard patients onto the program, ensuring compliance with all relevant regulations and protocols.* Conduct monthly touchpoints with patients following standard scripts to monitor progress and address any concerns.* Maintain accurate data records and submit timely dashboards and de-identified reports to the client.* Close the loop with HCPs regarding participating patients and share approved dashboards with them.* Design and implement program introduction brochures, customized landing pages, and other promotional materials as needed.* Provide support and guidance to HCPs, clinics, and patients as required to ensure the smooth operation of the program.Requirements:* Bachelor's degree with 2 years in life science, hospital management, dietician* Previous experience in a similar role, preferably in the healthcare sector.* Strong communication and interpersonal skills, with the ability to build rapport with a diverse range of stakeholders.* Excellent organizational abilities and attention to detail.* Knowledge of healthcare regulations and data privacy requirements.* Ability to work independently and manage multiple tasks simultaneously.* Proficiency in Microsoft Office Suite and other relevant software applications.* Flexibility to travel within the assigned geographical area as needed.
