This is a full time, permanent role working with our teams in Cranbourne and within the Cardinia area.

About the Role Monash Health Mental Health Services provide a range of community bed-based and specialist services covering Casey, Cardinia, Greater Dandenong, Kingston and Monash Local Government Areas. Our Pakenham and Cranbourne Community Mental Health service provides a public community service within the catchment area, catering for referred adults with a diagnosed psychiatric illness.

The Physical Health in Mental Health Nurse Practitioner (PHMHNP) or Candidate (PHMHNPC) will work across sites at the Cranbourne and Pakenham Continuing care team. In this senior nursing role, you'll provide complex, patient centred intervention and treatment planning for our Adult mental health population, assisting them to manage aspects of their physical health, including nicotine dependence.

You'll work collaboratively with a fantastic group of people who are inclusive, passionate and share a vision to provide excellent, evidence-based care for patients and their families.

About You: You can readily show your commitment to Quality Improvement, research and workforce development You are passionate about your own personal and professional growth Your verbal and written communication skills are exceptional What you need: Endorsement as a Nurse Practitioner or enrolled in or able to commit to enrolling in the Masters of Nursing Nurse Practitioner program Current AHPRA registration Valid Driver's Licence Demonstrated experience of working in an advanced mental health nursing role Previous experience of working in a Community mental health team will be highly regarded.
