At Southern Health Partners our medical team experiences the entire range of patient situations from detox to restorative progress, medical emergencies, chronic care management, mental health and well-being, all while educating the patient on how to move forward and continue to improve their overall health even after their release. Location: Door County Jail Open Position: Social Worker, Counselor (LPC) or Psych RN Schedule: 1 -8 hr Day shift per week 8a - 4:30p Qualified Mental Health Professionals will: Work closely with SHP Medical Director and/or SHP Medical Team Administrator to coordinate mental health treatment within the facility, as well as outside the facility. Provide mental health in-service training to the medical, psychiatric and security staffs, as needed, and/or requested. Intervenes and defuses mental health emergencies within the facility. Attend commitment hearings, if needed (or per SHP contract with facility). Develop treatment plans and goals for individual inmates. Document all patient contacts. Maintain confidentiality of medical records. Verify past patient mental health history, including verification of any psychotropic medications used. Prepare regularly scheduled reports and special reports as necessary. Work with nursing and mental health staff for post-release referral. Interested in Learning More About Life At SHP?
