We are seeking a Clinical Pharmacist to join a dedicated healthcare team in Swansea. This role involves working collaboratively with GPs, nurses, and other healthcare professionals to ensure optimal patient care. - Manage and review patient medication plans.
- Provide expert advice on the safe and effective use of medicines.
- Conduct medication reviews and reconcile medicines post-hospital discharge.
- Educate patients on proper medication use and potential side effects.
- Support GPs in achieving QOF and prescribing targets.
- Qualified Pharmacist with GPhC registration.
- Strong communication and organisational skills.
- Independent Prescribing qualification (desirable).
