Locum Community Physiotherapist – Falls Prevention Team Location: Romford Car Driver Essential ⏳ Contract: Full-Time | Community-Based Are you an experienced Physiotherapist with a passion for working in elderly care, community settings, and falls prevention? We are looking for a skilled professional to join our Falls Prevention Team, supporting patients at high risk of falls to improve their mobility, safety, and independence. Key Responsibilities:

• Conduct community visit s to assess and treat housebound patients with mobility and balance concerns.
• Perform triage assessments to identify urgent, priority, and suitable cases for referral to the BHR Falls Service.
• Deliver tailored physiotherapy interventions aimed at reducing fall risks, improving strength, and enhancing quality of life.
• Collaborate with multidisciplinary teams to develop and implement holistic care plans.
• Educate patients and caregivers on strategies to prevent falls and maintain mobility.
• Travel to patient homes to provide direct care within the community setting.

What We’re Looking For:

• HCPC-registered Physiotherapist with experience in elderly care, community rehabilitation, and falls prevention.
• Strong clinical skills, including assessment, care planning, and delivering person-centered interventions.
• Excellent communication and teamwork abilities.
• A valid UK driving license and access to a car (required for this role).
