*Salary Range: *£38,092 - £42,389 per annum, plus Market Supplement of £2,000 pro-rata per annum Make sure to read the full description below, and please apply immediately if you are confident you meet all the requirements. *Work Location: *Totton, near Southampton, with travel across the area, hybrid working (home, community and office based)*Hours per week: *37 (part-time hours will be considered)*Contract Type: *Permanent Join our friendly, supportive team as a qualified *Occupational Therapist* with HCPC registration. You’ll conduct home visits to lead holistic assessments, plan and review interventions. You’ll provide support across our multi-disciplinary Reablement Service, a strengths-based short-term service helping the people we care for maintain or regain independence in daily activities. *What you’ll be doing:** Leading on the assessment, planning and reviewing of complex case work. * Conducting person-centred functional analysis and interventions to help patients maintain or regain independence in daily activities. * Collaborating with a diverse team, including Occupational Therapists, practitioners, carers, sensory staff and business support. * Working independently, providing expertise and support to staff, informed by social care legislation and clinical knowledge. * Our service operates seven days a week and there will be a requirement to work weekends on a rotational 1:8 basis. *What we’re looking for:** Occupational Therapy qualification and professional registration with HCPC. * Experience in assessment, planning and reviewing of case work. * Ability to work independently and collaboratively, escalating cases as required. * Commitment to demonstrating our values and behaviours. * An understanding of the statutory duties of local government in social care and the social care model. * Some experience of supervising staff. *Why join us?** Join the largest care provider in Hampshire. * Opportunities for experienced and newly qualified Occupational Therapists.
