About the role As a Service Leader in our organization, you will play a pivotal role in enhancing the quality of life for seniors in the community. Your focus will be on delivering person-centred care, ensuring that clients receive the support they need to maximize their choice and control over their services. You will lead and mentor a dynamic team, setting a standard for best practices in case management, particularly within the Commonwealth Home Support Programme (CHSP). This role manages a CHSP caseload across the Brisbane South region, with the flexibility to work in one of our many offices in the Brisbane South, Scenic Rim or South West region. This Role is a 12-months fixed-term opportunity. Key Responsibilities:

  • Lead and guide a team of support workers towards creating positive outcomes for clients by embedding person-centred practices.
  • Provide regular supervision, constructive feedback, and performance evaluations to support workers and other direct reports in achieving their professional goals.
  • Emphasize person-centred goals and outcomes, engage in reflective practice, and effectively manage associated risks.
  • Ensure that clients’ personal care, support, and health needs are met in accordance with applicable standards and regulations within the service stream.
  • Cultivate and maintain strong relationships with key internal and external stakeholders.
  • Actively promote the services offered by the program to clients, their families, and the broader community to enhance awareness and cooperation.
  • Oversee the operational management of services, which includes budget management, program development, compliance adherence, and record-keeping.
  • Manage vacancy rates to ensure service continuity and availability for clients in need.
Skills & Experience:
  • Leadership experience in a community or human services organisation.
  • Knowledge of funding standards, guidelines, and legislation.
  • Strong communication, leadership, and problem-solving skills.
  • Strong written and computer skills for the creation of plans, programs, reports, and correspondence.
  • High level communication, influencing, liaison and negotiation skills.
  • Established problem solving and conflict management skills.
Essential requirements:
  • Must have experience in Aged Care Case Management highly desirable.
  • Must hold a relevant qualification or a Diploma in Aged Care, Disability, Community Services or equivalent.
  • Must hold or be eligible to obtain a Working with Children's Card (commonly known as Blue Card).
  • Must hold or be eligible to obtain a NDIS Working Screening Card (commonly known as Yellow Card).
  • Must hold or be eligible to obtain a current National Police Certificate.
  • Must hold a Full QLD Driver's Licence.
