Job Summary: The Team Leader is an integral part of the support team, responsible for meeting the practical, physical, and emotional needs of the individuals we support within the service. Through a key worker system, they will design individual support programmes and coordinate the delivery of these packages by a group of Support Workers and themselves. About The Role Key Accountabilities- Plan, coordinate, and implement the safe transition of new individuals into the service and community where necessary.
- Liaise with the families of the people we support and relevant members of multi-disciplinary teams, community groups, and voluntary bodies to promote their needs.
- Plan, develop, and implement individual and group programmes of support to enhance and maximize the capabilities and independence of the people we support in the service.
- Coordinate ongoing assessment and review regularly through the completion of suitable assessment models.
- Support the physical, emotional, and personal needs of the people we support, ensuring that work is individual-focused and upholds the principles of respect, privacy, dignity, fulfilment, independence, and choice.
- Ensure that medication is held, stored, and administered according to Praxis Care’s Medication Policy and current statutory and legislative procedures.
- Provide effective leadership, supervision, and management to Support Workers, including monitoring time and attendance and participating in any capability, disciplinary, and grievance procedures.
- Participate in learning and development as necessary within the organization while identifying and addressing personal development opportunities for themselves and Support Workers.
- Ensure that adequate records are created and maintained, and necessary reports are produced in line with organizational policy and statutory requirements.
- Support the manager with regulatory compliance concerning all regulating bodies.
- Participate in the company appraisal and staff review process.
- Adhere to and model the company values, behaviours, and competencies at all times.
- Engage in formal and informal supervision as required.
- Carry out all duties in accordance with Praxis expectations, regulations, and policies.
- Take on additional duties of the role and other required duties as necessary.
- Work the necessary hours to fit the needs of the service.
- Where the role requires sleep-ins, a minimum additional payment of €101.60 per night applies.
- 2 years’ experience in a health and social care setting and a minimum of QQI Level 5 in Health Care or equivalent.
- Experience in mental health or learning disability services.
- Full valid driving licence and access to own vehicle (If a disability precludes an individual from holding a driver’s licence, it should be stated how they can meet the mobility requirement of the post).
