The cardiac and vascular program performs more heart surgeries and procedures than any other facility in the province. Position Summary Acute Medicine at the Hamilton General Site encompasses 8 West and 8 South which are both Clinical Teaching Units. 8 West provides care to acute adult medical patients as well as alternate level care patients. The interdisciplinary team collaborates to provide high quality care for patients with acute medical diagnoses, such as myocardial infarction, diabetic ketoacidosis, pneumonia, endocarditis and chronic medical diagnoses, such as heart failure, COPD, coronary artery disease throughout the various stages of their illnesses. Using a client-centered approach, the Registered Nurse (RN) provides leadership in the coordination and evaluation of nursing care of patients with multisystem medical conditions awaiting transition to the community setting. The RN works collaboratively with interdisciplinary team members and community agencies to coordinate care and education of patients and their families. Through evidence-based clinical practice and strong leadership in healthcare delivery, the RN sets the standards of excellence in the care of patients at HHS. The RN establishes and implements education plans with and for patients and families and acts as an accessible and knowledgeable resource for patients and their families. Schedule Work Hours Rotating days, evenings, nights, weekends, and holidays Qualifications Current Certificate of Registration with the College of Nursing of Ontario as a Registered Nurse Current Basic Cardiac Life Support (BCLS) certification Completion of HHS’ Critical Practice Orientation (CPO) training or relevant step down course preferred ECG monitoring education and experience preferred Recent, within the last two (2) years related nursing experience in acute medicine and/or geriatrics Demonstrated ability to utilize problem solving and critical thinking techniques Demonstrated ability to work in a fast paced, high turnover environment Demonstrates strong interpersonal, communication and teamwork skills Demonstrates critical thinking and decision making in assessment, problem identification, problem solving and management of adult and geriatric medicine patients, including rehabilitative approaches to patient care Evidence of commitment to professional practice and ongoing professional development, demonstrated leadership ability and organizational skills Demonstrated proficiency in working with patients, families, peers and multidisciplinary healthcare teams in an effective, professional, caring manner Willingness to participate in Continuous Quality Improvement (CQI) and innovation Hamilton Health Sciences fosters a culture of patient and staff safety, whereby all employees are guided by our Mission, Vision, Values, and Values Based Code of Conduct.
