Job details
Hiring Home Healthcare Patient Coordinator with a minimum of 3 years experience and connections in the community (hospitals/clinics) to provide service referrals when needed. The ideal candidate should be aware of community resources, possess a high level of cultural competency, and be involved in community outreach. It is vital that you are familiar with health information technology and how to complete the required documentation, including POCs, Medicaid CANS, and progress notes. You should be able to work well with a team and directly with patients and their families. Excellent communication skills are essential. When implementing Medicaid Managed Care, you must have the ability to review and collaborate with other managed care organizations.
Education:
Bachelor of Science in Nursing.Language Requirements:
