As Admission Discharge Transition Manager (ADT) in Lotus Care, you will play a pivotal in ensuring the delivery of high-quality residential / respite support services for individuals with intellectual disabilities and autism. You will support dedicated teams in providing personalised care, fostering inclusivity, and facilitating community engagement for our service users. Your responsibilities encompass overseeing the initial referral process by building strong relationship with our referring partners and ensuring a smooth and seamless well planned admission discharge and transition process that will be a positive experience for our service owners and their loved ones. Your role involves close collaboration with the Person in Charge (PIC) and Regional Operations Manager to uphold Lotus Care's values of partnership, dignity, respect, and choice. The ADT role will be involved with the service owners up to 3 months post admission. This position offers a unique opportunity to make a meaningful impact in the lives of individuals with diverse needs, promoting their active participation in shaping their support plans and ensuring their voices are heard throughout their journey with Lotus Care. Key Responsibilities: Job Objectives: The person appointed will be required to work closely with Office of the CEO , DOS, RSMs, MDT, Operations, HR , Finance & Facilities Manager to ensure there is a seamless process from receipt of referral up to admission and thereafter for an initial period of 3 months post admission. Attendance and coordination of referrals meetings. Coordination of referral assessments Participate in assessment of new referrals via Teams with MDT/Operations and thereafter carry out in person assessments in the home/current service with the required MDT / PIC. Following assessment make recommendations to referrals committee in relation to service needs/requirements to support new admissions. Engage with MDT/Operations in relation to compatibility of new admissions with other residents to determine what best suits needs. Management of all documentation in relation to referrals and ahead of placement offer/admission as relevant. Keep all referring partners (HSE / Tusla) updated in relation to status of referral/placement options. Chair Lotus Care Referrals Committee Keep families updated in relation to status of referral/placement options. Schedule/manage reviews of 1:1 supports in line with funding approvals and contribute to review process. Where admissions are confirmed attend pre-admission DOS / RSM and liaise with Operations in relation to transition process, HR in relation to staffing requirements, MDT in relation to environmental and equipment needs, Facilities in relation to any building modifications required.
