Mon to Fri (Office Hours) Community settings Various openings available (Part Time & Full Time) As a Care Befriender (East & West region) (Community Setting), you will be responsible for the following duties: Conduct community outreach to engage with elderly individuals within the designated service boundary. Assess the needs of elderly clients through home visits and interactions, identifying those requiring additional support or services. Provide befriending services to frail, vulnerable, or isolated elderly individuals, offering companionship and emotional support. Conduct regular home visits to ensure clients' well-being and provide any necessary assistance as part of the care plan. Refer elderly clients to appropriate social services, healthcare, or other support services based on their needs. Ensure that clients receive timely and adequate assistance by liaising with external service providers. Recruit and engage volunteer befrienders to assist in outreach and befriending activities. Organize and conduct training sessions for volunteers, equipping them with the necessary skills to provide monitoring and appropriate referrals. Maintain accurate records of client visits, interactions, and care provided. Prepare and submit reports, including visitation notes, client records, and relevant data or statistics required by authorities or management. Work closely with the Centre Supervisor to implement care plans, monitor client conditions, and adjust services as needed. Participate in regular meetings to review the progress and effectiveness of the outreach and befriending services. Perform any additional duties as assigned by the reporting officer or management, ensuring smooth operation of the befriending and outreach program. Requirements: Minimum Diploma Holder in any field of studies. Some relevant experiences in elderly befriending service / social community care sector. Volunteering experience will be considered as well.
