Job details
Responsible for the counselling of patients on the use of CPF (Central Provident Fund) Medisave accounts, applying governmental financial assistance schemes to assist patients in need. Explain chronic care management packages available. Process case documents and write social reports to facilitate financial applications. Conduct peer audit checks on Medicare Assistance. Ensure proper documentation. Provide assistance and support to the Medical Social Workers in social service case management for patients in need at the polyclinic. Perform any other responsibilities as assigned by the supervisors. Requirements Possess a Diploma / Higher NITEC / A-Level. At least 1 to 2 years of customer service experience. Pleasant personality with excellent communication and interpersonal skills.
