About us:
The Central Queensland Hospital and Health Service (CQHHS) Nurse and Midwife Navigator Service provides a centralised Central Queensland-wide model of delivery, with Nurse Navigator positions operating from community care settings, based at Gladstone, Biloela (Banana), Emerald (Central Highlands) and Rockhampton (Capricorn Coast, Mount Morgan, Woorabinda).About the role:
Working within the four pillars, this role aims to facilitate a safe and efficient consumer journey, through the reduction of service fragmentation, educating and empowering individuals and coordinating consumer care. The role is intended to transcend access multiple specialities, support nurse-led service models and augment existing models of care within a facility or clinical area. The Nurse Grade 7 is a Registered Nurse who:- Demonstrates an advanced level of autonomous clinical and professional practice.
- Demonstrates an advanced level of decision making in collaboration with a multi-disciplinary team.
- Undertakes operational leadership.
- Works collectively to apply the principles of clinical governance.
- Applies professional and clinical expertise in collaboration with nursing and midwifery and multi-disciplinary stakeholders.
- Takes responsibility for enacting strategies that supports a work-based culture than promotes and supports education, learning, research, and workforce development.
About you:
- Current registration as a Registered Nurse with the Australian Health Practitioner Regulation Agency (AHPRA) and a current annual practising certificate are mandatory for this position.
- While not mandatory, a relevant post graduate qualification, or working towards would be well regarded.
- Demonstrated experience and ability to work across multiple specialities, transcending the traditional silos of acute, subacute, general practice, residential aged care and community care.
What we offer:
Salary information: Total remuneration value of up to $164,212 p.a. is comprised of:- An hourly rate between $66.85 - $72.58 p.h.
