About You! We are seeking dedicated ACAS Assessors to join our multidisciplinary Health team working across metro Melbourne’s Southern & Eastern suburbs. Allied Health Assessors will work within a primary health care framework to assess the needs of older persons and appropriate younger people with disabilities, within the region. Facilitate access to appropriate community and residential services through a multi-disciplinary team approach. Preferred candidates will have:

  • Experience in ACAS assessments - essential
  • Qualifications in a Healthcare background (e.g., Registered Nurse, Social Worker, Physiotherapist) and registered with AHPRA or relevant professional body.
  • Ability to work independently and as part of a team.
  • Demonstrated experience in conducting comprehensive assessments.
  • High level of computer literacy skills.
  • Ability to follow guidelines relevant to the position and demonstrated problem-solving capacity.
  • Current Australian driver’s license with own reliable, fully insured vehicle.
  • Valid National Criminal History Check and Working with Children Check.
What You’ll Do! You’ll play a major role in conducting assessments in the community, working both independently and as part of a broader team. Your duties will include, but are not limited to:
  • Completing face-to-face assessments to determine older Australians’ eligibility for support within My Aged Care.
  • Reviewing previously assessed clients.
  • Undertaking client-centred support planning using a wellness and reablement approach.
  • Liaising with referral sources and community support organisations to assist our clients in obtaining the home supports they require.
  • Ensuring accurate and timely completion of assessments and documentation, utilising the My Aged Care online portal.
