MAJOR DUTIES AND RESPONSIBILITIES SPECIFIC (80%) Be certified and trained in skills as an ACP Facilitator and ACP Trainer. Facilitate ACP discussions with patients/individuals, families, and NHS. Document these discussions and the wishes of the patient/individual in the appropriate forms (these may be hard copies or electronic forms). Update and liaise with primary care physicians on patient’s advance care plan. Coordinate between patients/individuals, relatives, and healthcare team for ACP facilitation and to follow-up with the necessary appointments for them. Mentor, train and guide junior ACP Facilitators in ACP case management, documentation, and communication skills. Develop and improve ACP training curriculum and programmes e.g. Advocacy, General ACP and Preferred Plan of Care (PPC) Facilitation courses. Be involved in grief bereavement work and provide psychosocial support to patients and where appropriate, follow up with referrals to spiritual and/or psychiatric support. Develop and be involved in the spiritual care work and support for ACP patients and their family members. GENERAL (20%) Support or conduct ACP awareness talks, road shows, and training. Work in collaboration with key stakeholders in the evaluation and quality improvement processes for the ACP program. Participate in conducting audits, research, and surveys related to ACP. Where necessary, staff will be nominated as co-investigators of research projects and are expected to be involved in data collection, abstract, and paper writing etc. JOB REQUIREMENTS EDUCATION, TRAINING AND EXPERIENCE A Basic Diploma with 5 to 6 years of working experience. Ability to speak in different languages and dialects would be advantageous. Strong communication skills, motivated, resourceful and a good team player. Experience in ACP facilitations for 2 to 3 years in any setting. PROFESSIONAL LICENCE Nursing or allied health (social work etc.) background preferred. ABNORMAL PHYSICAL WORKING CONDITION Staff may be required to travel to patients’ homes, other step-down care units or other locations as needed to conduct ACP discussions. Staff may be exposed to patients and their family members of various psychological and emotional states and who possess varying personality traits. #J-18808-Ljbffr
