Leading Non-Chronic Care Function- Develop and implement strategies to achieve 100% members engagement.
- Define and communicate health outcome targets to the team.
- Collaborate with internal and external stakeholders to implement evidence-based care protocols that drive improved health outcomes.
- Continuously evaluate and adjust care plans to achieve 100% compliance with outcome targets.
- Develop and implement the Non-Chronic Care strategy.
- Support the development and implementation of a comprehensive overall medical engagement strategy.
- Utilize various communication channels, including phone, digital platforms, health contact center, medical library, and digital care telehealth, to engage with patients.
- Support in managing the cost allocated for medical engagement initiatives, ensuring cost effectiveness and efficiency.
- Ensure compliance with healthcare regulations and industry standards.
- Implement quality assurance processes to maintain high standards of care.
- Stay current with digital healthcare trends and technologies.
- Implement and manage digital health initiatives to enhance patient engagement and outcomes.
- Apply corrective actions when deviations are identified.
- Establish and monitor key performance indicators (KPIs) related to member engagement, health outcomes, and cost reduction.
- Take proactive measures to address any deviations from targets and implement corrective actions.
- Utilize data analytics & other metrics to track program performance and provide regular reports to stakeholders.
- Make data-driven decisions to optimize program effectiveness.
- Identify specific healthcare challenges and opportunities within the population.
- Develop profiles or personas of the target population to better understand their unique requirements.
- Utilize data analytics to monitor the effectiveness & overall engagement.
- Get involved in data-driven decisions to optimize engagement efforts.
- Build and lead a high-performing team of professionals specializing in medical engagement.
- Provide guidance, mentorship, and support to team members.
- Supervise and lead a team of healthcare professionals, including doctors and provide guidance, support, and training to ensure team members meet performance targets.
