What You’ll Need
- A bachelor’s degree in human services .
- A valid driver’s license .
- A willingness to learn—we’ll provide all the training you need!
A Day in the Life
As an Aftercare Case Manager, you’ll support children and families who have achieved permanency through reintegration, adoption, custodianship/guardianship, or aging out of care. You’ll:- Focus on risk management and help families maintain stability during the aftercare period.
- Provide crisis intervention and work with public and private agencies offering supportive services.
- Maintain accurate documentation and reports.
- Coordinate community and educational services for families.
- Meet with families in-person to support reunification and resolve any concerns.
