*Job Profile: Licensed Practical Nurse - Medical Coordinator (Disability Management Team)* HHS is seeking a Licensed Practical Nurse (LPN) to join our team. This role involves assigning intakes to the Case Management team, securely uploading and managing confidential medical information, and submitting and coding invoices with accuracy. Additionally, the successful candidate will provide support for various Case Management tasks and responsibilities, contributing to the seamless operation of the team. Attention to detail, organizational skills, and the ability to manage multiple priorities are essential for success in this position. This role involves managing a high volume of intakes and administrative work and requires exceptional attention to detail, discretion and a commitment to maintaining the highest standards of confidentiality and accuracy. Responsibilities include maintaining organized records, processing medical documentation efficiently, and collaborating with healthcare professionals to support seamless information management. Experience in occupational health is considered an asset for this role. Candidates must have a minimum of 3 years of clinical experience in acute care or a hospital setting, managing both complex and routine medical issues. This background ensures the ability to handle a wide range of medical situations with expertise and professionalism. Strong critical thinking and problem-solving skills are essential for this role. The ideal candidate will demonstrate the ability to analyze complex situations, make sound decisions under pressure, and develop effective solutions to a wide range of challenges. The successful candidate must possess the ability to effectively interact and collaborate with individuals across various corporate roles, including managers and field supervisors.
