Job Title: Salaried GPwER Dermatology Salary: Dependent on experience AL: 30 days per annum (pro-rated as appropriate) Hours: Sessions between 8:00am – 6:30pm, Monday - Friday). Potential overtime (including weekends). Responsible to: Clinical Lead and Service Manager Key Relationships: 32 General Practices, 4 PCNs, NWL ICB, Local Minor Surgery Services, Acute Specialist Service Departments (Level 4), Specialist Skin Cancer services, Secondary care specialties such as plastic surgery, rheumatology and diabetes. BACKGROUND WHO WE ARE - Healthcare Central London Healthcare Central London Ltd (HCL) is the GP Federation owned by our 32 General Practices covering the Central London (Westminster) area. The organisation supports 4 Primary Care Networks (PCNs). We operate several NHS contracts on behalf of our PCNs including a Community Dermatology Service; Community Cardiology Service and an out-of-hospitals provision which is sub-contracted to our 32 General Practices. Our vision is to be recognised as a leading GP provider network, run by clinicians for the benefit of our local population and practices. We aim to further diversify our income by exploring commercial joint ventures and expanding our research team in the coming years. Community Dermatology Service Healthcare Central London’s Community Dermatology Service (CDS) offers non-emergency care close to home for patients with a range of chronic skin conditions, including eczema, extensive acne and psoriasis. The service is available to any patient aged 16+ who is registered with one of HCL’s member Practices. The overarching aim of CDS is to provide an innovative, high quality, consultant-supported GPwER service to Westminster patients. ROLE PURPOSE Due to expansion of our services, we are seeking a dedicated and experienced GP with an Extended Role (GPwER) in Dermatology, to join our dynamic clinical team our CDS clinics. The GPwER will play a key role in delivering high-quality community services to our patients and collaborating with other healthcare professionals to promote patient well-being. Main duties of the job You will be committed to maintaining clinical continuity for patients. You will participate in learning events, audits, and other agreed-upon projects. Your willingness to understand and improve practice systems, learn new IT systems, and support continuity of care will be highly regarded. Collaboration with colleagues, mutual learning, and respect for multidisciplinary teams are essential. Staying updated with new guidance and addressing any time-management challenges will ensure smooth operations. Job Responsibilities

  1. Patient Consultations: Conducting face to face and/or remote appointments. Assessment and Diagnosis - Conducting thorough assessments and making accurate diagnoses based on clinical presentation, history taking, physical examination, and relevant investigations.
  2. Treatment and Management: Developing and implementing appropriate treatment plans, recommendations for prescriptions, providing advice on self-care and lifestyle modifications, signposting and arranging referrals to other services when necessary.
  3. Patient Triage: Participating in patient triage processes to prioritise and handle appointments based on clinical urgency and need.
  4. GP Education: Conducting or participating in webinars aimed at Westminster referrers.
  5. Patient Education and Empowerment: Providing patients with clear information about their health conditions, treatment options, and self-management strategies.
  6. Joint Clinics: Participating in clinics with a Consultant Dermatologist.
  7. Clinical Team Meetings: Participate in quarterly journal clubs and meetings aimed at clinicians with CDS.
  8. Clinical decision making: Making informed clinical decisions to ensure patient safety and well-being.
  9. Collaboration with healthcare team: Working collaboratively with members of other services.
  10. Documentation and Record Keeping: Ensuring accurate and timely documentation of patient encounters.
  11. Quality Improvement and Clinical Governance: Engaging in continuous quality improvement activities.
  12. Professional Development: Pursuing opportunities for professional development.
  13. Adherence to Policies and Protocols: Following established policies, protocols, and clinical guidelines.

Person Specification Qualifications and Experience Essential
  • Qualified GMC Registered GP
  • MRCGP
  • Accredited as a GPwER
  • Entry on the Performer's List
  • Proven experience in handling a busy community care caseload
  • Clinical experience of 2+ years
  • Experience of clinical triage
Desirable
  • Training in Clinical Leadership or Management
  • Teaching / Training experience
  • Experience using S1, Accurx
  • Delivery of clinical audit
  • Experience of working in Westminster general practice
Knowledge Essential
  • Knows when to make independent clinical decisions and seek advice from senior clinicians.
  • Working knowledge and a commitment to team approach and multi-disciplinary working.
  • Understanding of clinical risk management and clinical governance.
  • Knowledge of various IT systems and good IT proficiency.
  • Understanding of the need for strict confidentiality.
