Our services help clients to remain safe and independent in their homes.We are a fast-growing, family-owned business, seeking a highly positive team player to join our Greater Logan office based in Shailer Park.We are seeking an experienced, enthusiastic and highly motivated Care Coordinator to coordinate, manage and administer the delivery of care services to our clients. Our clients are a mix of Home Care Package, NDIS and Private Pay across all age groups.This role is responsible for:Managing and coordinating services for home care package clientsDelivering outcomes/goals for our clients based on their care/NDIS planMonitoring HCP budgets for clients depending on level of Government assistanceWorking collaboratively with our clients to coordinate supports and services to improve their quality of lifeConducting client care assessments and care planning with a person-centred care approach in alignment with goals/outcomes and within budget restraintsUnderstanding, managing and working within government contract requirementsSupervision and management of Care StaffScheduling appointments and in-home services to appropriate staff based on discipline, availability and geographical requirements with our unique carer matching at the forefront of decisionsCoordinating the provision of quality person-centred care according to the client's needs, service/care plans and available funding in accordance with budgetBuilding relationships with local stakeholdersMaintaining a positive attitude with the ability to work in an ever-changing environmentExhibiting high emotional intelligenceParticipating in a rotating roster of weekday and weekend 'on call' with other staff membersQualificationsEither:Cert III/Cert IV (Aged/Disability Care) with a minimum of 1 year of experience in a coordination role ORRegistered Nurse (RN) in community nursing with 1-2 years of current AHPRA registration OREnrolled Nurse (EN) in community nursing with 1-2 years of current AHPRA registrationExperience within the aged care/disability/mental health in a community settingUnderstanding of Aged care & Disability standards and consumer-directed careFirst Aid, current CPR and National police checkWorking with children Blue Card (or willingness to obtain)Driver's licence - AustraliaReliable vehicleHigh level of computer literacy and understanding of technologyUnderstanding of clinical governance and continuous improvementPossessing a 'can-do' positive attitude and ability to work well in a team environmentPreferred SkillsPrevious experience with rostering is highly desirableExperience with Microsoft Word and ExcelAn outgoing personality with high energy levels, good communication skills and personable telephone mannerAbility to problem-solve and think quickly under pressureAbility to multitask with a strong attention to detail while keeping calm and focusedExcellent networking and consultation skills to develop new referral sources and relationships with appropriate external organisationsExcellent customer service skills with exceptional verbal and written communication skillsSound knowledge and understanding of home care services and guidelines
