ConnectWell Community Health is seeking a Registered Nurse (RN) Certified Diabetes Educator (CDE) for 21 hours per week (0.6 FTE) to join our inter-professional team. This position offers a comprehensive benefit package and HOOPP. *Position Summary: *The Certified Diabetes Educator (CDE) Registered Nurse (RN) promotes the health of individuals, families and the community ConnectWell Community Health serves. The CDE RN works with the inter-disciplinary team to provide individual and group education to adults on prevention and management of chronic health conditions including prediabetes and type 2 diabetes. The CDE RN participates in the development, implementation, monitoring and evaluation of health promotion programs and services for individuals, families and the community that are affected by chronic health conditions including diabetes. The CDE RN must be aware of, understand, and adhere to the Standards of Practice set out by the College of Nurses of Ontario and the current Canadian Diabetes Association Clinical Practice Guidelines. *Qualifications:*• Bachelor of Nursing Degree or an equivalent combination of education and experience • Current and unrestricted registration with the College of Nurses of Ontario • Certified Diabetes Educators certification or commitment to acquire • Three to five years of nursing experience in health and diabetes education • Excellent health assessment and clinical skills, understanding of chronic diseases including diabetes • Experience in group facilitation for adults • Demonstrated ability to work effectively as a member of an inter-disciplinary team. • Experience in the development and delivery of health education and health promotion programs. • Excellent communication skills, both oral and written. • Excellent organizational, time management, prioritization, and interpersonal skills. • Experience and understanding of health equity and rural communities *Reporting Relationship: *The successful candidate will report to the Primary Care, Manager For a complete job description, please visit our website: *HOW TO APPLY: *Qualified candidates are invited to submit a cover letter and resume detailing their qualifications and interest in the position.
