Clinical Liaison Responsibilities The primary purpose of the Clinical Liaison is to be accountable for the immediate referral response process to achieve performance goals. The clinical liaison will utilize the information provided through the electronic medical system and the client relation management system to complete a clinical assessment. The clinical liaison will review and evaluate prospective resident referrals by the approved clinical capabilities, bed availability, integrated eligibility, and sex offender status to provide an immediate response to the referral partner, and effectively communicate the determination with the community. Clinical Liaison Requirements
  • Graduate of a Licensed Practical Nurse or Registered Nurse program.
  • Active, in good standing, RN or LPN license in the state of practice.
  • Minimum of two years of prior experience in a Clinical Liaison or similar Clinical Assessment position within a long-term care setting.
  • Intermediate to advanced computer skills, including prior experience working with an EMR or EHR.
  • Excellent written and oral communication skills.
  • Ability to make independent decisions when circumstances warrant such action and be able to handle confidential information per protocol.
  • Looking for an individual who has patience, tact, and a cheerful disposition; also a candidate that shows enthusiasm, flexibility, and personal integrity.
  • Willingness to work harmoniously with different members of a team.
  • Leadership, organization, and time management skills required.
