Duties/Responsibilities: Assists Unit Manager to: Complete and ensure resident assessments are accurate, up to date and changes communicated as required Plan, coordinate, manage and evaluate the resident's care plan working with the resident/substitute decision maker, physician, nursing and interdisciplinary team and contracted service providers Under the direction of the Medical Director and/or Nurse Practitioner, process and follow orders. Assess and accurately reports resident response to care in a timely manner Prepare for scheduled care conferences and participates in the conferences as applicable Responds to resident/family concerns and ensures appropriate action is taken and/or concerns communicated to the Unit Manager and/or Director of Care Responsible for the safe and accurate administration of medications and treatments. Additionally, to assess and document response to treatment and report any incidents related to medication administration to the Unit Manager and Medical Director Provides education and support to residents and families by maintaining an open, knowledgeable rapport. Communicates changes in resident's condition as necessary, in an ethical manner which follows privacy and confidentiality protocols Responds to and manages significant changes in resident's condition and or medical emergencies; provides emergency first aid Provides supervision and direction to PSWs while promoting a multidisciplinary team approach to care and service Assists to evaluate the performance of PSWs annually and as needed Requirements: Current registration as Registered Practical Nurse in the province of Ontario Experience working in a long term care environment and with unregulated care providers preferred Strong written and oral communication, critical thinking, care coordination, time management and customer focus skills Demonstrated specific knowledge /skill in current geriatric nursing care and standards preferred Computer proficiency in PointClickCare and experience with MDS-RAI or other electronic documentation system required Wound care experience required For all new Grads or RPN's who have not worked in the field for 6 months or more, we have been approved to offer an incentive through CCPN. This incentive will pay you up to $25,000 for a 24-month commitment of full-time hours. Since Maxville Manor is located in a remote/rural location, you would also qualify for special grants which can be applied towards any student loans you may have incurred during your education.
