We are seeking an Admissions Coordinator, on a fixed term basis, to join our facility at Kingsley, a 41-bed transitional care program (TCP) facility specifically designed for after-hospital care where our clients have time to recuperate before returning home or awaiting other suitable long-term accommodation.

The Admissions Coordinator TCP primary role is to liaise with the Department of Health WA; TCP Central Coordination Team (North and South); and the site multidisciplinary team to coordinate admissions and discharges for TCP, ensuring clients meet the admissions criteria as per TCP guidelines, in consultation with the Care Manager. The Admissions Coordinator is responsible for the timely management of transition care bed movements to ensure maximum occupancy is maintained.

This is a fixed term permanent part-time position may suit an exisiting Regiestered Nurse with TCP experience.

The position will be working 45 hours per fortnight, Monday to Friday.

To be successful in this role you will have: An Australia Health Practitioner Regulation Agency (AHPRA) registration as a Registered Nurse or Enrolled NurseDemonstrated ability to assess and evaluate client care needs within a nursing and allied health framework.Demonstrated understanding of the complex clinical environment within aged care.Excellent communication (both written & verbal) and interpersonal skills that delivers effective leadership, builds, and develops teamwork and establishes successful working relationships with stakeholders.Demonstrated ability to organise and priorities tasks, work effectively under pressure, problem solve and meet deadlines.Working knowledge of Occupational Safety and Health legislation; Aged Care Act (1997) and/or Disability Services Commission Standards (1988).Recent acute, subacute, restorative or GEM experience in an adult hospital setting (desirable).Evidence of up-to-date flu vaccinationSatisfactory NDIS Worker Screening check Brightwater can offer you: The ability to salary package  - meaning you can legitimately pay less tax and increase your take-home payOur unique staff benefits  program, including access to BrightwaterPlus which gives you discounts with over 370 retailersFree on-site parkingAbout Brightwater With a track record that dates back to 1901, care is at our core.
