This is a maternity relief position and should the permanent incumbent require an early return from maternity leave, this contract of employment will cease with two weeks notice. Maternity relief positions can be for a period of up to 12 months, with the possibility of continued employment for up to a further 12 months for maternity leave reduced hours, therefore there are no claims to permanency.

Join our team as a Clinical Nurse Specialist within the Transitional Aged Care Program! As a pivotal figure in the admission process, you'll leverage your clinical expertise and communication skills to ensure seamless transitions, coordinate care effectively, and lead innovative nursing practices.

Please note that this role is a job share maternity relief cover arrangement.

About You

You're a highly skilled and experienced Registered Nurse with current AHPRA registration and relevant post-qualification experience, making you an ideal fit for our team. Your adept communication and technology skills enable you to provide top-tier specialty care and drive innovation in nursing practice. Whether collaborating with multidisciplinary teams or working independently, you consistently deliver outstanding results. Your expertise in complex care and clinical leadership, coupled with your adherence to professional standards, ensures excellence in service delivery. With a current NSW Driver's License and a readiness to travel across the Central Coast, you're prepared to make a meaningful impact in our community.

The Transitional Aged Care Program provides short term support and low intensity therapy for older people at the conclusion of a hospital episode.
