This is an incredible opportunity to be part of a dynamic team that has a client focus whilst supporting each other as a team.In this role you will provide responsive case management to young people between the ages of 15 and 25 years of age, while effectively building relationships with clients and encouraging self-determination and participation. As well as that, you will be supporting the development and growth of young people's lives through the collaboration with internal and external stakeholders.You will successfully Support young people in line with Youth Services model of care and associated FrameworksBuild effective relationships with clients that encourage self-determination and participationProvide ongoing assessment of client's issues, safety, needs and circumstances from initial intake to exitDevelop individual case plans in collaboration with clients including a range of interventions that address the clients needsAssist clients to access support services and establish community supports including referrals to appropriate specialised agenciesMaintain accurate up to date client records including case notes using TSA information systemsThis is full time permanent position, located in Ingle Farm, SA.
