Are you a Diabetes Specialist Nurse (DSN) looking for an opportunity to work with young people? We are looking for an experienced DSN to join our team to support our NHS England Transition and Young Adult Care Pilot. This post has been uniquely created as a secondment to offer opportunity to develop experience and skills caring for young people living with type 1 diabetes aged 16-25 years old. The role is an outreach post that is community based. You will be responsible for delivering diabetes advice, care and support for young people in the cohort that require complex case management. You will work collaboratively with a fail safe officer, youth worker and mental health services. Candidates should be expert clinicians and educators as a key responsibilities will be the delivery of structured education. Successful candidates should be passionate about supporting young people to live well with diabetes and improve health outcomes.

Detailed job description and main responsibilities

You will work across paediatric, adult and GP services in the community. As the Transition Outreach Nurse, you will be responsible for providing a flexible model of care to improve access to clinical care and reduce inequalities for 19–25-year-olds (This may include working outside 9-5 Monday to Friday) You will directly have responsibility as a keyworker for a caseload of young people aged 18 approx. In the final year in paediatric services, you will work towards achieving best practice and the transition standard ……of 2 joint appointments…. You will support the young person at diagnosis and at times of admission to hospital to harness the potential that the young person may be able to focus on health improvement.You will with other TraSS team members identify and support those young people struggling with engagement with diabetes services, insulin omission and those attending A and E with diabetes related emergencies and mental health issues. Qualifications, knowledge and experience

  • Registered nurse on relevant part of the NMC register
  • Level 7 Post Grad Cert (Min 60 Credits), Master’s Degree within relevant speciality or equivalent knowledge to be assessed through evidence of relevant speciality training, experience or portfolio
  • Specialist training / Qualification in area of practice
  • Provide evidence of leading learning in practice
  • Evidence of experience and completion of all band 6 competencies within relevant speciality
  • Specialist clinical skills and experience within specialist area
  • Evidence of change Management, leadership and innovation
  • Clinical experience at Band 6 or equivalent within relevant speciality
  • Provide evidence of supporting colleagues to promote learning in practice providing an excellent learning environment for all
  • Provide evidence of supporting/ managing and supervision of a range of learnings in practice
  • Counselling / advanced communication skills qualification / training
  • Act in the role of Practice Assessor or supporting the progression of learners / supporting challenges as needed
