Our client, A leading healthcare facility is currently seeking passionate individuals to join our team as Home Care Nurses. As a Home Care Nurse, you'll have the unique opportunity to provide personalized care and support to patients in the comfort of their own homes, helping them maintain their independence and improve theirquality of life. Working Conditions: Key Responsibilities:
  • Conduct comprehensive assessments of patients' health status and develop individualized care plans.
  • Provide skilled nursing care, including wound care, medication administration, and monitoring vital signs.
  • Educate patients and their families on disease management, medication adherence, and self-care techniques.
  • Collaborate with interdisciplinary healthcare teams to coordinate and optimize patient care.
  • Maintain accurate and timely documentation of patient assessments, interventions, and outcomes.
  • Provide emotional support and compassionate care to patients and their families.
Qualification and Skills:
  • Valid nursing license (DOH or active DHA).
  • Minimum of 2 years of nursing experience, preferably in home care or a similar setting.
  • Proficient in conducting comprehensive assessments and developing care plans.
  • Strong clinical skills, including wound care, medication administration, and monitoring vital signs.
  • Excellent communication and interpersonal skills.
  • Compassionate, patient-centered approach to care.
  • Ability to work independently and as part of a collaborative team.
  • Reliable transportation and willingness to travel to patients' homes within the designated area.
  • Certification in Home Health Nursing (HHCN) or similar specialty.
  • Experience working with geriatric or chronically ill patients.
