Our Rehab and Nursing Centers have been providing services in West Michigan for 20+ years. Our residents range in age, diversity and acuity levels. Key/Unique Facility Details:

  • Our two-floor facility was constructed in 2016. It has a 76 bed Post-Acute Rehabilitation unit and an 88 bed Long Term Unit.
  • Each floor has designated units that focus on patient specialties such as: General rehab, neuro rehab, traumatic brain injuries, cancer, and other specialties.
LPN Position Summary: Under the direction of the RN / Mid-level Provider / Physician and in collaboration with other members of the health care team, contributes to the assessment, planning, implementation, and evaluation of patient care. RN Position Summary: Delivers nursing care responsibly and accountably as described professionally by the American Nurses Association (ANA) and legally by the State of Michigan. Demonstrates competency through the critical thinking model known as the nursing process. This includes assessment, diagnosis, outcomes identification, planning, implementation, and evaluation. Upholds the standards of professional performance as described by the ANA. This includes ethical practice, culturally congruent practice, communication, collaboration, leadership, education, evidence-based practice and research, quality of practice, professional practice evaluation, resource utilization and environmental health. Strives to achieve optimal outcomes.  Weekend Option Program:
  • Weekend RNs/LPNs will be required to work 48 out of 52 weekends per calendar year.
  • Weekends are defined as Saturday and Sunday for day shift and Friday and Saturday night for night shift.
  • RNs/LPNs can request up to 48 hours of PTO on their weekend shifts as well as 48 hours of exchanging a weekend shift for a weekday shift, for a total of 96 hours ‘off’ of the weekend requirement. This would be given in accordance with unit guidelines with allotted PTO as well as peak times, etc.
  • RNs/LPNs may request 1 holiday off per calendar year that falls on the weekend. The RN/LPN would only work the holiday if it fell on the weekend, they will not be moved off a weekend to work on a holiday.
Qualifications
  • Associate Degree Nursing or LPN Certification - Required
  • RN or LPN license in State of Michigan - Required upon hire
  • Basic Life Support (BLS) 90 Days Required
