  • Record cases information, complete accurately all necessary forms and produce statistical ;
  • Coordinate and provide skilled communication, timely, effective, efficient, equitable and patient-centered.
  • Handle case assignments, draft service plans, review case progress and determine discharge and transition plans.
  • Help patients achieve wellness and autonomy.
  • Facilitate multiple care aspects (Case coordination, information sharing, etc) with the Nursing Supervisor.
  • Help patients make informed decisions by acting as their advocate regarding their clinical status and treatment options.
  • Develop effective working relations and cooperate with the medical team through the entire case management process.
  • Take the extra mile and interact with patients to keep track of their progress and to ensure high satisfaction.
  • Promote quality and cost-effective interventions and outcomes.
  • Assess and address motivational and psychosocial issues.
  • Adhere to professional standards as outlined by protocols, rules, and regulations.
  • Collect the required documents from patients and provide them to the Nursing Supervisor.
  • Inform the Nursing Supervisor about new referrals or patient's related complaints.
  • Acceptance of new referrals according to the Scope of Service.
  • Accompany the Nursing Supervisor and clinicians to the patient's place of residence for assessments.
  • Coordinate with the Nursing Supervisor and other healthcare facilities about admission or discharge of the patient.
  • Coordinate with patient relatives for the requirement of any documents or change in the services or any update in the nursing schedule.
  • Provide feedback from patients/ relatives to the Nursing Supervisor and the Management regarding the service provided or about staff attitude.
  • Inform the patient regarding changes in schedule or service as per the Nursing Supervisor information.
  • May be required to perform other job-related duties as may be assigned by the Mangaer from time to time.
  • Coordinate with physicians or hospital concerned staff if there is a need to update the medical report according to the Nursing Supervisor recommendations.
  • Conduct at least one monthly visit to patients in their place of residence accompanied by the Nursing Supervisor.
  • Resolve patient related conflicts.
  • Maintain confidentiality of patient's record.


Required qualifications

Master's in Public Health
