Job Summary

 

The post holder will provide and maintain a high standard of nursing care for patients as well as providing nursing assistance to the doctors and other members of the primary health care team. Supported by senior nurses within the practice, they will deliver care within the boundaries of their role, focusing upon nursing treatments, supporting patients to be healthy, monitoring of long-term conditions, health prevention and screening activities. S/he will work collaboratively with the general practice team to meet the needs of patients, practice key performance indicators and targets, QOF, support the delivery of policy and procedures and provide nurses leadership as required.

 

CLINICAL RESPONSIBILITIES

ï‚· To provide high quality patient care and treatment.

ï‚· Participate in setting standards for care and auditing clinical outcomes.

ï‚· Promote and participate in maintaining a safe comfortable and therapeutic environment.

ï‚· Participate in developing strategies and promoting health promotion programs for patients andcarers and facilitate screening clinics.

ï‚· Take responsibility for the safe storage and administration of drugs and equipment.

ï‚· Provide nursing care in a variety of settings by holistic assessment, implementation and evaluation of programs of care as appropriate.

ï‚· Participate in setting and monitoring standards to ensure effective care ensuring delivery of evidence based practice.

ï‚· Provide information, advice and support to patients face to face and via the telephone

ï‚· Involvement in team discussions to ensure "best practice"

Administration of immunisations/vaccines (Child & Adult) Including Zoladex Injection

Health promotion and lifestyle advice

Assisting with chronic disease management.

 

ORGANISATIONAL SKILLS

 

To maintain accurate records and data using the Emis Web computer clinical system

To work as an effective team member and promote a multi-agency collaborative approach to health care ï‚· Maintain the established reporting documentation and communication system.

To ensure efficient evaluation and monitoring.

Report and record all clinical incidents.

 

COMMUNICATION

 

Demonstrate sensitive communication styles to ensure patients are fully informed and consent to treatment.

Communicate with and support patients receiving "bad news".

Communicate effectively with patients and carers, recognising the need for alternative methods of communication to overcome different levels of understanding, cultural background and preferred ways of communicating.

Utilise communication skills to support patients to adhere to prescribed treatment regimes.

Anticipate barriers to communication and take action to improve communication.

Maintain effective communication with individuals and groups within the practice environment and with external stakeholders.

 

DELIVERING A QUALITY SERVICE

 

The Surgery aims to deliver the highest standards of care to its patients at all times. This post will be integral to the delivery of that aim and the appointee will be expected to:

 

ï‚· Recognise and work within own competence and professional code of conduct and other requirements as regulated by the Nursing and Midwifery Council (NMS) and to ensure familiarity with and adherence to those requirements.

ï‚· Produce accurate, contemporaneous and complete records of patient consultation, consistent with legislation, policies and procedures.

ï‚· Prioritise, organise and manage own workload in a manner that maintains and promotes quality.

ï‚· Deliver care according to the NSF and the National Institute for Clinical Excellence (NICE) guidelines and evidence-based care.

ï‚· Assess effectiveness of care delivery through self and peer review, benchmarking and formal evaluation.

ï‚· Participate in the maintenance of quality governance systems and processes across the organisation and its activities.

ï‚· Utilise the audit cycle as a means of evaluating the quality of the work of self and the team, implementing improvements where required.

ï‚· In partnership with other clinical teams, collaborate on improving the quality of health care, responding to local and national policies and initiatives as appropriate.

ï‚· Evaluate the patient's response to health care provision and the effectiveness of care.

ï‚· Support and participate in shared learning across the practice and wider organisation.

ï‚· Participate in the management and review of patient complaints, and identify learning from clinical incidents and near-miss events using a structured framework ( root cause analysis).

ï‚· Participate in the performance monitoring review of the team, providing feedback as appropriate.

ï‚· Understand and apply legal policy that supports the identification of vulnerable and abused children and adults, being aware of statutory child/vulnerable adult health procedure and local guidance.

ï‚· Work within policies relating to domestic violence, vulnerable adults, substance abuse and addictive behavior, and refer as appropriate.

 

TEAM WORKING

 

ï‚· Understand own role and scope in the organisation and identify how this may develop over time.

ï‚· Work as an effective and responsible team member, supporting others and exploring the mechanisms to develop new ways of working.

ï‚· Where involved in delegation of tasks, delegate clearly and appropriately, adopting the principles of safe practice and assessment of competence of those taking on delegated duties.

ï‚· Ensure clear understanding and utilisation of referral mechanisms within the practice.

ï‚· Accept delegation from other nurses, prioritise own workload and ensure effective time-management strategies are embedded in own practice.

ï‚· Work effectively with others to clearly define values, direction and policies impacting upon care delivery. ï‚· Participate in team activities that create opportunities to improve patient care.

ï‚· Participate in and support local projects as agreed with the practice management team.


PERSONAL DEVELOPMENT AND TRAINING

 

ï‚· Understand own role and scope, and identify how this may develop over time.

ï‚· Be actively involved in professional development and maintain a professional profile in accordance with the Nursing and Midwifery Council (NMC) requirements.

ï‚· Act at all times within the boundaries of the NMC Professional Code of Conduct.

ï‚· Maintain up-to-date records are per policies and NMC guidance.

ï‚· Maintain a current NMC registration

ï‚· Take responsibility for own development, learning and performance and demonstrate skills to other members of staff when required.

ï‚· Attend any external training course thought to be appropriate to the role.

 

MANAGEMENT OF RISK

 

ï‚· Manage and assess risk within the areas of responsibility, ensuring adequate measures are in place to protect staff and patients.

ï‚· Monitor work areas and practices to ensure they are safe and free from hazards and conform to health, safety and security legislation, policies, procedures and guidelines.

ï‚· Ensure safe storage, rotation and disposal of vaccines and drugs is undertaken. Where appropriate, oversee the monitoring, stock control and documentation of controlled drug usage according to legal requirements.

ï‚· Undertake mandatory and statutory training.

ï‚· Apply infection-control measures within the practice according to local and national guidelines.

ï‚· Apply policies that reduce environmental health risks, are culturally sensitive and increase access to health care for all.

 

OTHER DUTIES

 

ï‚· Any other duties, as agreed with the Partners to meet the needs of the practice.

ï‚· To engage in the wider activities of the organisation where possible and appropriate.

ï‚· Practice policies and procedures relating to the protection of personal and sensitive data must be adhered to at all times and only divulged to authorised persons.

ï‚· The privacy, dignity and beliefs of patients, carers and colleagues must be respected.
