Home Health Care Staff Nurses (Home Visit Nurses)

Qualifications:
- Bachelor’s degree in nursing.
- 2 years of experience in the same Field.
- Dataflow is a must.
- Saudi license holder is a plus.
- Fluent in English.
- Conduct initial and ongoing assessments of patients’ health status, including physical, psychosocial, and functional abilities.
- Develop and update individualized care plans in collaboration with the healthcare team, considering patient preferences and goals.
- Administer medications, injections, and treatments as prescribed, monitoring for side effects and effectiveness.
- Perform medical procedures such as catheterization, wound care, and intravenous (IV) therapy.
- Manage and monitor chronic conditions (e.g., diabetes, heart failure) and provide education on self-management strategies.
- Teach patients and their families about disease processes, medications, and lifestyle modifications to promote health and prevent complications.
- Provide guidance on nutrition, exercise, and other aspects of wellness.
- Coordinate care with physicians, specialists, therapists, and other healthcare professionals to ensure comprehensive treatment.
- Facilitate communication between family members and the healthcare team.
- Regularly assess and document patients’ progress, changes in condition, and responses to treatment in electronic health records (EHR).
- Ensure compliance with regulatory standards and policies and regulations.
- Offer empathetic support to patients and families, addressing emotional needs and facilitating access to social services if necessary.
- Provide counseling on coping strategies and resources for mental health support.
- Identify and respond to emergencies promptly, utilizing appropriate interventions and resources.
- Develop and implement emergency care plans tailored to individual patient needs.
- Participate in quality assurance and improvement initiatives to enhance patient care and service delivery.
- Engage in ongoing education and training to stay updated with best practices and advances in home healthcare.
