Nurse Manager (Home Healthcare Team)

The Nurse Manager manages and guides community nurses and care staff in order for them to provide holistic clinical care to clients and their caregivers at home to a certain standard, to allow clients to keep well in the community and minimize need for admission to the hospital. Responsibilities Clinical Quality and Assurance Ensure staff care plans and follow-ups are carried out in accordance to expected standards to deliver good clinical outcomes. Plan developmental needs and competencies of clinical staff based on operational requirements. Review and maintain clinical SOPs and policies with Programme Manager/Director to ensure compliance with MOH service requirements for non-residential ILTC services. Review and follow up on escalated clinical incidents with Programme Manager/Director. 2. Team Management & Preceptorship Provide direction and guidance on staff performance and conduct to ensure clarity of staff expectations. Provide direction and guidance on care planning of complex cases and/or cases in alignment to the aims of the Home Healthcare team as set out by Programme Manager/Director. Delegate caseload appropriately in accordance to staff competency. Ensure staff licenses, trainings and competencies are kept up to date. 3. Project Management Assist the Director/Programme Manager in planning, implementing and evaluating the Programme’s operations and programmes to meet the clinical needs of home personal care, interim caregiving, home medical and home nursing services. Provide guidance and supervision to clinical staff in carrying out quality improvement projects. Requirements Education/ Training/ Certification Degree in Nursing SNB Registration Experience At least 5 years of relevant experience Personal Attributes Strong clinical skills Excellent interpersonal skills in working with both internal and external stakeholders #J-18808-Ljbffr
