Job Description

  1. To ensure proper handing over and taking over with writing detail note in over register as per protocols.
  2. To make sure timely dispensing and distribution of medication, ensuring patients profiles and CPOE for rational drug use, to prevent drug interactions and to promote rational drug use.
  3. To contact the physician for in-accuracy of medication order and document intervention (accepted or rejected) accordingly; if intervention is rejected for some reason, it will be shared with the clinical team to present to the formulary committee.
  4. In case of non-availability of medicine, contact physician for switching to available alternate/substitute. If not, then process for Non-formulary arrangement through NFI policy.
  5. Ensure all orders are timely processed and prompt delivery of medications to the Nursing unit.
  6. Monitor and manage inventory stock levels, manage when needed.
  7. Prepare and forward departmental requisitions to warehouse.
  8. Receive supplies from warehouse, physically check batch, expiry quantity, packaging, cold chain if applicable, and shelve the stock in proper location at specified temperature.
  9. Make sure light sensitive, cytotoxic drugs, high alert and look alike and sound alike drugs are placed accordingly.
  10. When duty assigned at OPD pharmacy where medicines are dispensed directly to patient, pharmacist must counsel the patient and respond to patients’ queries.
  11. When duty assigned at oncology/cytotoxic stations, pharmacist will do the following:
  • Making patient protocols: Check patient profiles, baseline labs of patient like CBC, LFT, and Creatinine before making chemotherapy protocol & preparation of regimen; three copies of protocols must be generated: one sent to nursing, one to patient, and one for Pharmacy record.
  • Double check patient protocol and counter sign.
  • Before issuance and preparation of drugs protocol prepared, CPOE order double check should be done by pharmacist also at chemo processing station.
  • Intrathecal drugs like Methotrexate, Cytarabine, & Hydrocortisone are issued in early morning (before dispensing regular chemo) to avoid errors or as specified by the policy and protocol.
  • Patient profile review to ensure right diagnosis, age, and protocol/phase; once chemo regimen prepared, pharmacist will counter check/recheck each drug and counter sign.
  • Hand over chemo of all patients to nursing for administration; for any reason drug is not administered/refund is also documented in excel sheet for record as per protocol.
