Social Worker-(2300002I) Description JOB DUTIES STATEMENTS: Duties and Responsibilities: Addresses social concerns of patients Performs psychosocial assessment and counseling therapy to patients, their families Provides follow-up services to appropriate patients and families. Participate in preparation of the treatment plan, especially for patients with psychosocial issue, and their treatment requires certain resources such as family support, transport, finance etc. Participate in preparation of the discharge plan to help patients to return to their home environment with optimal functioning condition Responds to referrals placed for inpatients, outpatient (RDU, special needs and kidney transplant patients) and ER, as per policy. - Follows up on patients for whom initial assessment is done when indicated. Helps families cope with loss and grief situations (Code Mercy and Code Blue). Provides support to DNR (Do Not Resuscitate) patient's family. Assists patients facing financial needs by making case study and referring them to the appropriate charity channels. Contacts patient who refused admission or refused continuation of case to ensure they are in stable condition and to understand reasons for DAMA, as per policy. Provides help and special care for patients with special needs like the terminally ill, handicapped, victims of abuse or domestic violence and those patients presenting with social problems, which are not psychiatric in origin. Coordinates the reporting process of the abuse and domestic violence cases to MOH and HRSD, and report the cases to the Violence and Abuse Protection Committee. Be part of the multidisciplinary discharge planning team. Coordinates periodical visits of charity institution to the inpatient. Issues Priority to Them Cards and parking legibility stickers for Special Need patients as per policy. Helps in communicating with clergy and interpreters if needed for patients. Coordinates with the accounting department to define the financial covers & limits. Attends committees & meeting as recommended. Other duties as assigned within the scope of responsibility and requirements of the job. Arrange family / physicians meeting including preparation, attending the meetings Screening and support to the patient and family in ICU, CCU, NICU, PICU Social Workers shall an updated of Interpreter, clergies and sign language list to interpret for patients and their families. Accumulates and maintains extensive information on community resources and programs available to meet the patient's needs and make this information available to the patient and family. social workers in kidney transplant conduct psychosocial evaluation, presents data and addresses any problems identified to transplant committee and should provide education to patients regarding expectations post-transplant for medications and insurance coverage Demonstrates compassionate communication skills while providing support and care to the patients as "the patient is the center of our care". Complete mandatory education as per the requirement at least one month prior to expiration. Maintain up to date knowledge on information relating to quality and safety, accreditation, policy management and document management systems.
