Curtin Heritage Living is a leading provider of aged care services, and we're looking for a dedicated and compassionate In-Home CHSP Coordinator to join our growing team! Shine Community Care, part of Curtin Heritage Living, has been providing exceptional community support services within the aged care sector for over 40 years. As a not-for-profit organisation, we are dedicated to offering local, personal, and quality care to our clients and community. About the Role As one of our In-Home CHSP Coordinators, you'll be the primary contact and advocate for our valued clients. You'll build strong relationships, understand their unique needs, and create individualised care plans that promote wellness and reablement. - Experience: Demonstrated experience as an In-Home Coordinator, with a strong understanding of the Commonwealth Home Support Programme (CHSP).
- Client Focus: A genuine passion for working with older people and a commitment to providing exceptional customer service.
- Communication Skills: Excellent communication and interpersonal skills to build rapport with clients, families, and colleagues.
- Care Planning Expertise: Proven ability to create and implement person-centred care plans that focus on wellness and reablement.
- Industry Knowledge: Sound knowledge of aged care funding models and experience managing budgets.
