If you have initiative, are resourceful, engage easily in teamwork, and most importantly, want to make a difference in healthcare, we want to hear from you!Position SummaryThe Quality Improvement Lead operates at an operations management level, fostering collaborative partnerships with operational managers within Mosaic Primary Care Network (MPCN). This role will report to the Manager of the Reporting Evaluation and Data Integration department.This pivotal role involves joint analysis of data to proactively uncover opportunities for refining both clinical and non-clinical programs. By closely engaging stakeholders, the role drives the co-creation, execution, and oversight of improvement initiatives geared towards enhancing operational efficiency and quality standards—ranging from acceptability and accessibility to appropriateness, effectiveness, efficiency, and safety—across MPCN programs. This collaborative effort aligns seamlessly with the objectives of Mosaic PCN, Calgary Zone, and Provincial targets.The QI Lead will collaborate with cross-functional teams, including clinical and Non-clinical managers and their staff, to leverage best practice, enhance and optimize processes, identify unwanted variance, and support compliance with program standards, and support continuous quality improvement.This involves the identification and rectification of deviations, upholding compliance with program benchmarks, and nurturing the culture of continuous quality advancement. This role is committed to integrating quality metrics, patient-reported experience measures (PREMs), and patient-reported outcome measures (PROMs) to assess program efficacy and amplify patient contentment, thereby cultivating a comprehensive culture of quality improvement.This role will coach clinical and non-clinical managers and evaluation team members regarding QI process, and coordinate for all components of quality improvement practice including: planning, designing, implementing, monitoring, adapting, and sustaining.Key Responsibilities Strategic Collaboration for Clinical Programs: Engage proactively with clinical programs at Mosaic Primary Care Network (MPCN), actively contributing to process development, implementation, and the continuous refinement of practices.Partner closely with program stakeholders to align the aspirations and objectives of the program with MPCN's overarching mission and vision.Contribute actively to program planning, evaluation, and the tracking of performance, fostering a collaborative approach to improvement. Empowering the Medical Home Support Program: Play a key role in supporting the seamless implementation and ongoing enhancement of the Medical Home Support Program within MPCN.Collaborate effectively with the REDI Manager, Program Manager Medical Home Support, and program stakeholders to ensure program adherence, compliance with MPCN's strategic direction, and alignment with Zone and Provincial guidelines.Utilize data analysis, encompassing quality metrics, PREMs, and PROMs, to assess and elevate the patient care experience within the framework of the Medical Home model. Insightful Data Analysis and Communication: Gather, analyze, and interpret program-specific data, including quality metrics, PREMs, and PROMs, to pinpoint areas for improvement and systematically track program outcomes.Identify data sources, design data collection plan and analytical plan, deploy analytics tools and methodologies adeptly to evaluate program efficacy, patient experiences, and clinical outcomes.Perform advanced analytical techniques include statistical modeling, marching learning, and artificial intelligence, effectively translate analytical concepts into plain language for non-technical stakeholders.Collaborate closely with the Reporting, Evaluation, and Data Integrations (REDI) team, as well as the Clinical Informatics Analyst role, to develop and maintain data dashboards and comprehensive reports that effectively communicate insights to MPCN stakeholders.Create comprehensive reports, manuscripts, publications, and impactful visual presentations to convey findings and recommendations adeptly to stakeholders within MPCN.Present the QI outcomes, on behalf the REDI team and MPCN, at internal and external meetings for a wide range of audiences including clinical and non-clinical leaderships, researchers, and relevant stakeholders. Deliver presentation about QI findings in local, regional, and national meeting. Driving Process Enhancement Collaborations: Actively engage with cross-functional teams to systematically evaluate and optimize processes within the Medical Home Support and Clinical Programs.Identify bottlenecks, inefficiencies, and potential avenues for streamlining and improvement, encompassing process efficiency, performance, documentation, technology integration, and the patient experience.Formulate and execute initiatives for process improvement, drawing upon established methodologies and tools such as Lean, Six Sigma, the Model for Improvement, and the Plan-Do-Study-Act cycle.Monitor and evaluate the efficacy of improvement efforts proactively, making necessary adjustments to achieve optimal outcomes. Ensuring Excellence and Adherence: Provide crucial backing to ensure compliance with program standards, guidelines, and regulatory mandates across clinical programs, the Patient's Medical Home, and other clinical initiatives.Stay update on industry-leading practices and evidence-based guidelines related to patient-centered medical homes, PCN-related primary care program delivery, and clinical program management.Play an integral role in the formulation and execution of strategies for quality improvement, meticulously tailored to align with MPCN's guidelines and aiming to elevate patient outcomes and uphold program standards. Engaging Stakeholders and Cultivating Understanding: Collaborate dynamically with program stakeholders, securing their active engagement and support for quality improvement endeavors.Deliver educational initiatives and training to team members on the nuances of quality improvement methodologies, process optimization, and data-driven decision-making.Promote a culture that champions continuous quality improvement and patient-centered care within the framework of the Medical Home Support Program at MPCN. Assessment and Insightful Reporting: Play a pivotal role in program evaluation efforts, encompassing comprehensive data collection, in-depth analysis, and impactful reporting of key performance indicators, quality metrics, and patient-reported assessments.Strategically deploy PREMs and PROMs to assess patient experiences, outcomes, and satisfaction levels.Participate actively in both internal and external audits, contributing constructively to the formulation of corrective action plans as required.Formulate comprehensive reports and compelling presentations that effectively communicate program outcomes, improvement initiatives, and patient-reported assessments to stakeholders within MPCN. Quarterly and Annual Report Support: Collaborates with evaluation team members to support quarterly operational reports, Board/Manager/Director KPI reports, and the Mosaic PCN Annual report evaluation section. Research Support When is necessary, provide support research request and projects including: data collection, analysis, interpretation, manuscript/publication writing.Qualifications and Requirements Education and Experience Bachelor's degree in healthcare administration, nursing, public health, health informatics, analytics, or a related field. A master's degree is preferred.Proven experience in health care quality improvement, program support, evaluation performance measurement and reporting, data analysis and data visualization.Strong analytical and problem-solving skills, with the ability to collect, interpret, and analyze complex data sets and visualize analytical outputs.Knowledge in knowledge translation or implementation science.Excellent presentation skills with the ability to deliver data information to a wide range of audiences, especially for people without technical background.Familiarity with quality improvement methodologies and tools, such as Lean, Six Sigma, or Plan-Do-Study-Act (PDSA) cycles.Knowledge of patient-centered medical home principles.Experienced in coaching and mentoring team members and clinical staff for quality improvement practice.Clinical Improvement Facilitator certification is desirable.Experience with provincial EMRs would be an asset.Change Management certificate is desirable (preferably Prosci).Hands-on experience in BI reporting tools: AWS Quicksight, Tableau, Power BI is required.Experience in data analytics using SQL, R, Python or SAS is highly desired.TheCore Competencieslisted below are identified by Mosaic leadership and should be demonstrated by all employees across the organization: Practice Communication Excellence (Communication) Plan and deliver oral and written communications respectfully and tactfully to make an impact and persuade an intended audience.
