PURPOSE:Promote, maintain, and improve individual and community health by assisting individuals and communities to adopt healthy lifestyles. Identify community needs prior to planning, implementing, monitoring, and evaluating programs designed to encourage healthy lifestyles. To develop health educational program for patients and their families. To assist in the development of patient educational materials and to plan and prepare group education sessions for patients aimed at level of understanding and educational abilities for the patient and/or caregiver.JOB DUTIES STATEMENTS:Duties and Responsibilities:Possesses the knowledge , skill and attitude to develop programs, educate patients and to assess the success of patient education programFully articulates all benefits of modified life style according to patient disease and the risks of non-complaintRecognizes all barriers to successful education and develops strategies to overcome barriersPrepare and distribute health education materials, including reports, bulletins, and visual aidsEducate both inpatients & outpatients at DSFH; liaises with the treating physician to resolve barriers and development of the individual educational planProvides assessment of referred patients to assess the needs for educational informationCollaborate with health specialists and community groups to determine health needs of the DSFH patients and community, providing the needed educationpatients regarding their medical equipment, medication administration; how to monitor vital signs and theof need to seek medical attention, any needed instruction on patient's health, safety and treatmentPrepares weekly reports of all activities including number patients visited inpatient and outpatient, educational sessions held and reports to Supervisor.Maintains professional relationships with patients, visitors and colleaguesPresents DSFH in a positive and professional manner in the community.Demonstrates compassionate communication skills while providing support and care to the patients as 'the patient is the center of our care'.Other Job Functions:Perform all duties and tasks in line with the Person Centered Care values.Adherence to all policies, plans and programs within the Fakeeh Care.Perform duties as per the code of conduct requirement and ethical standards as outlined in the HCM policies and guidelines.Complete mandatory education as per the requirement at least one month prior to expiration.Maintain up to date knowledge on information relating to quality and safety, accreditation, policy management and document management systems.Qualifications:Bachelor Degree in Health EducationMinimum 2 years of experienceEnglish proficiency - Medium levelLicensure / Registration from Saudi Commission for Health Specialties - EssentialCertified as Diabetic Educator
