Apply Now! Our Rehab and Nursing Centers have been providing services in West Michigan for 20+ years. Our residents range in age, diversity and acuity levels. Key/Unique Facility Details:

• Our two-floor facility was constructed in 2016. It has a 76 bed Post-Acute Rehabilitation unit and an 88 bed Long Term Unit.
• Each floor has designated units that focus on patient specialties such as: General rehab, neuro rehab, traumatic brain injuries, cancer, and other specialties.

LPN Position Summary: Under the direction of the RN / Mid-level Provider / Physician and in collaboration with other members of the health care team, contributes to the assessment, planning, implementation, and evaluation of patient care. RN Position Summary: Delivers nursing care responsibly and accountably as described professionally by the American Nurses Association (ANA) and legally by the State of Michigan. Demonstrates competency through the critical thinking model known as the nursing process. This includes assessment, diagnosis, outcomes identification, planning, implementation, and evaluation. Upholds the standards of professional performance as described by the ANA. This includes ethical practice, culturally congruent practice, communication, collaboration, leadership, education, evidence-based practice and research, quality of practice, professional practice evaluation, resource utilization and environmental health. Strives to achieve optimal outcomes.  Qualifications

• Associates Degree Nursing or LPN Certification - Required
• RN or LPN license in State of Michigan - Required upon hire
• Basic Life Support (BLS) 90 Days Required

Our typical shifts are as follows:

• 12 hours shifts nights: 6:30 p.m. to 7 a.m., Every third weekend
