*Job Summary* We are seeking a dedicated and compassionate Registered Practical Nurse (RPN) with a focus on Behavioral Support Outreach (BSO) to join our healthcare team. The ideal candidate will possess strong clinical skills and a passion for providing exceptional care to patients, particularly those with acute care needs, dementia, and other complex medical conditions. This role involves working collaboratively within a multidisciplinary team to ensure the highest standards of patient care. *Responsibilities** Provide direct nursing care to patients, including administering medications and treatments as prescribed. * Conduct comprehensive assessments of patients’ physical and mental health status. * Develop and implement individualized care plans based on patient needs and preferences. * Utilize medical terminology effectively in documentation and communication with healthcare team members. * Collaborate with case management teams to coordinate patient care and discharge planning. * Maintain accurate and timely documentation in electronic health records systems such as Cerner or Epic. * Assist in phlebotomy procedures as needed for laboratory testing. * Offer support and education to patients and their families regarding health management, particularly in nursing home settings. * Monitor patients for changes in condition and respond appropriately to emergencies or crises. *Requirements** Valid RPN license with relevant nursing qualifications. * Experience in acute care settings preferred, with a strong understanding of dementia care practices. * Familiarity with electronic health record systems such as Cerner or Epic is advantageous. * Knowledge of medical terminology is essential for effective communication within the healthcare team. * Ability to perform phlebotomy procedures competently is a plus. * Strong case management skills to facilitate patient-centered care coordination. * Excellent interpersonal skills with the ability to work effectively in a team-oriented environment. * Commitment to providing compassionate care while maintaining patient dignity and respect.
