Nurse Practitioner (NP) Bradford, ON Canada L3Z 4H2 Seeking Contract, Temporary, or Staff NP THE POSITION:

• We are seeking a dedicated Nurse Practitioner to join our team at a premier senior care facility, a top-quality independent living and assisted living facility focused on delivering exceptional, patient-centered care. In this role, you will oversee the clients in our 40-bed Transitional Care Unit, working directly with Southlake Hospital to ensure seamless care transitions and optimal health outcomes for our patients

• Oversee and manage care for the 40 patients in our Transitional Care Unit, in collaboration with Southlake Hospital
• Conduct comprehensive health assessments, diagnose conditions, and develop personalized care plans
• Prescribe medications, treatments, and therapies as appropriate
• Collaborate with multidisciplinary teams to ensure high-quality, holistic care for patients
• Serve as a primary resource for health education and counseling for patients and their families
• Ensure adherence to all regulatory health and safety standards in senior and transitional care

• Current Nurse Practitioner certification and license in Canada
• Proven experience in transitional or geriatric care, with a strong understanding of chronic disease management
• Exceptional communication, leadership, and collaboration skills
• Commitment to providing compassionate, high-quality care
