Role Title:Clinical Nurse Manager II: Discharge Co-Ordinator Job Reference Number:000794 Closing Date:Sunday, 10th November 2024 Purpose of the Role: The post holder will be responsible for the provision of the highest standard of nursing care to patients, manage all nursing and allied staff in the ward/unit, and be responsible for maintaining and developing professional standards. Key Duties and Responsibilities The role of the CNM II is to: Promote collaboration within the multidisciplinary team (MDT). Engage with key stakeholders to ensure hygiene standards are maintained for patients, staff, and visitors. Proactively manages quality and safety standards at local and Directorate level. The Clinical Nurse Manager II must adhere to Code of Professional Practice and Behaviours for Registered Nurses. Shows leadership potential to make an impact, influence, energise, and inspire others across the Directorate and pan organisation. Embraces change and acts to implement evidence based best practice where appropriate. Is aware of and understands the need to adhere to Hospital and/or professional standards, policies, and procedures, relevant to their area of practice. Communicates effectively in English language, written and spoken, as appropriate to job requirements. Develop and implement active discharge planning and manage safe discharges in partnership with medical, nursing, allied health, and any other staff member involved in a safe discharge. Lead in conjunction with the MDT with complex discharges. Communicate daily with ward staff in particular the CNM and or Nurse in Charge, maintaining close links between Bed Management and clinical practitioners. In consultation with the Bed Management team, medical, and nursing staff co-ordinate transfers and admissions. Act for the in-patient co-ordinator as required, also provide clinical expertise to the admissions staff. In consultation with the Bed Management team, have as far as possible, sufficient beds available each evening for admissions through the Emergency Department for both the COVID and Non-COVID pathway and critical care pathway. Facilitate the best use of beds for patient care, getting the right patient to the right bed. Actively engage with the virtual hospital model. Undertake any other functions appropriate to the Department to facilitate safe patient flow. Participate and cooperate with legislative and regulatory requirements in relation to Health and Safety as outlined in the Safety, Health and Welfare Act 2005 and the Hospital Safety Statement. Ensure own mandatory learning and training is kept up to date. * The above Role Profile is not intended to be a comprehensive list of all duties involved and consequently the post holder may be required to perform other duties as appropriate to the post which may be assigned to them from time to time and to contribute to the development of the post while in office. This Role Profile will be subject to review in the light of changing circumstances. Required: Registered General Nurse with The Nursing Midwifery Board of Ireland (NMBI), or eligible to register. 5 years post-registration experience inan acute hospital/relevant clinical areawithin the last 7 years. Demonstrate management experience throughout the last 2 years. Demonstrate I.T. skills including excellent keyboard skills. Experience in teaching and assessing the nursing and HCA team throughout the last 2 years. Desired: Management course. Bed management and discharge experience. Working knowledge of healthcare patient record systems. Working knowledge of PAS/EPR/BI.
