Responsibilities:Providescrisis intervention/rapid response and suicide risk assessmentservices as well as individual and group psychoeducation related tobetter management of health/behavioral health issues and to riskreduction of recidivism to criminal justice system under thesupervision of an independently licensedprofessional.Developsand coordinates community re-entry plans that address risk factorsassociated with recidivism to the criminal justice system.Maintainsknowledge of community resources and contacts to support patient seffective transition back to thecommunity.Completessuicide risk and bio-psychosocial assessments, community re-entryplans and other clinical reports/mental health forms in timelyfashion and under the supervision of an independently licensedprofessional.Obtainstreatment records and coordinate care with family, communityproviders, and others relevant to health care and community reentryplan.Completesand coordinates requests for mental health related Court OrderedEvaluation (COE) with community providers and testifies in court asneeded.RequirementsSupportingDocumentation/AdditionalQualifications:Copyof diploma/transcripts to support graduation with a bachelor sdegree in behavioral health, social services, or a relatedfield.Minimumone year of past work experience in thisposition.BLS/CPRcertificationSupporting Documentation/Additional Qualifications: Copy ofdiploma/transcripts to support graduation with a bachelor s degreein behavioral health, social services, or a related field.
