SummaryServes as a pharmaceutical healthcare provider by efficiently designing, recommending, activating, monitoring, and evaluating patient-specific pharmacotherapy to ensure optimal outcomes. Ensures implementation of the pharmaceutical care plan by assuring the accurate and timely distribution of medications to patients. Maintains liaison relations with medical and nursing staff. Serves as a clinical resource for other pharmacy personnel, medical and nursing staff, as well as other healthcare professionals.Essential Responsibilities and DutiesProvides in-depth pharmaceutical care to an adult or pediatric service by participating in patient care rounds with medical team, maintains outstanding skills as a clinical pharmacy practitioner and consistently demonstrates the value of pharmacy services within the integrated pharmacy model.Assists medical staff with the selection of drug therapy and patient monitoring.Reviews, verifies medication orders and interprets medications for clarity, accuracy, rationality compatibility, feasibility, and suitability for the patient's specific needs.Assumes responsibility for patient-specific pharmacotherapy plan to ensure optimal outcomes.Develops and evaluates therapeutic protocols/guidelines/pathways.Conducts daily systematic medication reviews for patients in other floors other than the floor covered by the clinical pharmacy specialist, whenever assigned.Participates in committees relevant to areas of service.Acts as drug information and education resource for patients, students, colleagues, medical and nursing staff, and other healthcare professionals.Documents therapeutic recommendations in the patient's medical record and pharmacy intervention formsConducts Medication Utilization Evaluation (MUE) projects.Provides real-time collaboration with nurses to create cost-effective medication care plan focusing on medication misadventure prevention.Detects, analyses, and reports adverse drug reactions as well as medication errors.Assures adherence to the departmental policies and procedures and provides monthly reports on clinical activities and other relevant workload statistics.Contributes to positive outcomes and meets the cultural expectations of the organization by exhibiting professional behaviour and expertise.Provides patient medication education as primary responsibility as assigned.Participates in the departmental performance improvement, educational, and other clinical activities.Follows all Hospital's related Policies and Procedures.Participates in self and others' education, training and development, as applicable.EducationPharm.D./Bachelor's Degree in Pharmacy with accredited by residencyprogram (PGY1 or PGY2) is required.Experience RequiredFour (4) years of clinical experience after completion of accredited GeneralPharmacy Practice Residency (PGY1) Diploma, or two (2) years of clinicalexperience after the completion of the Specialty Pharmacy Residency Program(PGY2) with Pharm.D./Bachelor's Degree is required.Other Requirements(Certificates)Current Registration from country of originand Saudi Commission for Health Specialties (SCHS) is required.Board of Pharmacy Specialties (BPS)certification is required for all candidates.A minimum of two publicationswithin the last 5 years in a peer reviewed journal indexed in Pubmed orequivalent is preferred.
